| SEEK allows organizations to identify frail or imminently (within
12 months) frail Medicare-age members. The SEEK survey system includes three
products – a Health Status Questionnaire, a Brief Health Questionnaire, and the
Frailty Wheel. Organizations may choose to purchase one product or a
combination of two or three to meet their individual needs. The Center for
Health Research (CHR) provides various types of support, including survey
mailing, data collection, and a customized database. To read more, select any
