This single-page 12-question survey, a cost-efficient re-fold self-mailer, is designed to update current information on Medicare-aged health plan members. The Brief Health Questionnaire takes only 5-10 minutes to complete, and there is no loss of accuracy compared to the Health Status Questionnaire. Most persons aged 65+ are not in poor health, so the survey includes questions of interest to healthy persons to ensure a high response. Frailty indicators are couched among innocuous questions of general interest such as meaningful relationships, sense of humor, and water drinking. Additional questions target geriatric syndromes such as:

  • Memory loss
  • Falls
  • Weight loss
  • Pain
  • Sleep problems

The instrument also includes informational “Points of Interest for Healthy Aging” to promote reflection on various aspects of safety and self-care.
